Free Flu Shot Clinic at CWASD!
The best way to prevent influenza or “the flu” is to be vaccinated.
Again this school year the Chetek Weyerhaeuser Area School District is working with the Barron County Public Health to provide a student flu shot clinic for students grades K4-12, at NO COST to families. The clinic will take place on Wednesday, October 23rd and operate during school hours. Barron County Public Health will allot their time to offer the clinic on site at both Roselawn Elementary and Middle/High School buildings.
Only the injectable form of the flu vaccine will be offered. If you wish to have your child vaccinated, please read through the vaccine information sheet (VIS), complete a consent form for your child, and return the consent form to the school nurse. A separate consent form is needed for each student who will receive the vaccine.
Consent forms are due back to the school nurse by October 17th , 2024. Only students with a signed parent consent form will receive the vaccine.
