A link to the 2018-19 Athletic/Co-Curricular Handbook is located directly to the right on your screen under C-W Athletic Forms. Links to all required forms are also located to the right on your screen under C-W Athletic Forms. New this year you will find the Student Handbook, the Athletic/Co-Curricular Handbook and the Concussion Agreement Form Signature pages combine in a simple two sided form. This will allow up to three family members in middle school and high school per sheet while asking for only one parent/guardian signature per side. All athletic forms must be completed before an athlete is eligible to begin practice. Please contact Carol Moon at 715 924.3137 x2003 or email cmoon@cwasd.k12.wi.us with questions regarding required forms and eligibility of an athlete.

Transportation Release Form: Parents may sign their child out with a coach at any away event. If a parent chooses to designate an adult(s) to sign their child out, this form must be completed and on file in the athletic office. A copy of this form will be given to coaches and be kept on file throughout the designated school year. If changes need to be made, please contact the athletic office. A copy of this form is to the right under C-W Athletic Forms.

The Chetek-Weyerhaeuser Athletics' Department is pleased to provide a notification system courtesy of rSchool Today - an electronic scheduling program. This service allows parents, student-athletes and anyone interested in CW Bulldog Athletics the opportunity to stay informed of schedule changes - cancelations, postponements, bus departure changes - through email and/or text alerts.

ImPACT TESTING OPPORTUNITIES
Athletic Eligibility Requirement

An athletic requirement for all student-athletes in grades 6-12 is a valid ImPACT test; before a student may begin practicing, they must have documentation of a valid test. Immediate Post-Concussion Assessment and Cognitive Testing, or ImPACT, is a tool utilized by the Chetek-Weyerhaeuser Area School District and Marshfield Clinic in order to maintain student-athlete safety and welfare.

ImPACT Features -

Measures player symptoms
Measures visual and verbal memory, processing speed and reaction time
Reaction time measured to 1/100th of a second
Assists clinicians and athletic trainers in making difficult return-to-play decisions
